A general, residential, building or roofing contractor, a building code inspector, or a Florida-licensed home inspector, with a dated signature. The form is not accepted without it.

Roofs older than 25 or 50 years

A soft roof, such as shingle or similar, older than 25 years needs documentation showing at least five years of remaining useful life. A hard roof, such as tile, slate, clay, concrete or metal, older than 50 years needs the same. A roof with less than five years of remaining useful life needs proof of a full-roof replacement, given to the inspector, before the roof is verified in good condition.

Wind mitigation features

This form does not verify loss mitigation features. The Uniform Mitigation Verification Inspection Form, OIR-B1-1802, is used for them.

Citizens updated this form to RCF-1 03 25 so that its roof condition requirements mirror those of the updated 4-Point Inspection Form.
