- Who signs it
- A Florida home inspector licensed under section 468.8314 with hurricane mitigation training, a building code inspector certified under section 468.607, a licensed general, building or residential contractor, a licensed professional engineer or architect, or another individual or entity the insurer recognizes.
- Required for
- Florida law requires every insurer to use this form when a policyholder submits it for a wind insurance discount, and to accept it as valid when a listed inspector signs it (section 627.711(2)(a), Florida Statutes).
- How long it is valid
- Up to five years, if no material changes have been made to the structure and no inaccuracies are found on the form.
